The fine print:
In order to protect your privacy and personal health information, all submissions will be reviewed prior to publishing. CVS/pharmacy will not publish any story that includes personal health information such as full name, social security number, birth date, brand name medications and certain protected health conditions. CVS/pharmacy will only publish stories that pertain to myCVS™ Story that do not reveal any personal health information.
Submissions will be reviewed within 48 hours of receipt during normal business hours of Monday–Friday 8:30 AM–5 PM ET.
By submitting your story, you are granting permission to CVS Caremark and its respective parents, subsidiaries, affiliates, successors and assigns, (collectively, “CVS Caremark”), the absolute right and permission to use or license material, in any manner and in any media.
Must be over the age of 18 to participate.
